During the order process, you will be required to enter your SMU Student ID Number, and select your Program/Campus.
IMPORTANT: Your SMU Student ID Number can be found on your SMU Statements (upper right hand corner). Be sure to add the letter 'P' and list all digits in the registration field (Example: P000012345).
IMPORTANT: Please retain your username and password so that you will be able to conveniently access, view, and print your information online.
24 hours after your drug screen registration, you will receive an email from www.castlebranch.com with your e-chain (electronic chain of custody form) registration form attached as a pdf document. Your 'e-chain' form has a bar-coded list of authorized lab locations to select from that are conveniently located near you. Please print the e-chain form and take it with you when you visit your selected lab.
NOTE: Your completed, confidential report will be (a) accessible to you online through your www.castlebranch.com account (b) automatically received and secured by the Office of the Vice President of Enrollment and Student Services. |
